Medical Treatment Of Unstable Angina

aİstanbul Üniversitesi Kardiyoloji Ensitütüsü,bDr. Siyami Ersek Göğüs Kalp ve Damar Cerrahisi Merkezi, İSTANBUL Acute coronary syndrome describes a clinical spectrum ranging from ST elevation myocardial infarction to non ST elevation myocardial infarction (NSTEMI) and unstable angina pectoris (UA). The syndrome is characterized by a ruptüre of a vulnerable coronary artery plaque, intraluminal thrombosis, embolisation and varying degrees of obstruction and perfusion. The severity of the obstruction and the amount of the effected myocardium determines the cilincal presentation. Total occlusion of a coronary artery which supplies blood to a large myocardial zone results ST segment elevation myocardial infarction, whereas the same occlusion results NSTEMI if extensive collaterals exist. Like this an incomplete occlusion of a ruptured plaque results ischemia microinfarction. By sensitive and spesific cardiac markers (troponins) the seperation of ischemia and infarction may be possible. If the cardiac markers elevate it is NSTEMI and if not it is accepted as UA.Keywords: Angina, unstable; angina pectorisTurkiye Klinikleri J Int Med Sci 2007, 3(15):61-69
