An Approach To Cardiac Arrhythmias During Pregnancy
Dr. Okan ERDOĞAN,a Dr. Feyza AKSUa
aKardiyoloji AD, Trakya Üniversitesi Tıp Fakültesi, EDİRNE Pregnancy can precipitate cardiac arrhythmias not previously present in seemingly well individuals. In pregnancy, significant changes occur in hormonal and hemodynamic state of women that make them more susceptible to arrhythmias. Treatment of maternal tachyarrhythmias in pregnancy is a major clinical issue. Management of maternal arrhythmias in pregnant women is similar to that in nonpregnant but a special consideration must be given to avoid adverse fetal effects. No drug therapy is usually needed for the management of supraventricular or ventricular premature beats, but potential stimulants, such as smoking, caffeine, and alcohol should be eliminated. Although no drug is completely safe, most are well tolerated and can be given with relatively low risk. Some antiarrhythmic agents, such as propranolol, metoprolol, digoxin and quinidine, have been extensively tested during pregnancy and have proved to be safe. When drug treatment fails or is not indicated because of the haemodynamic instability of the patient, direct current cardioversion can be used. Keywords: Pregnancy, arrhythmias, antiarrhythmic drugsTurkiye Klinikleri J Int Med Sci 2005, 1(50):69-71
