aKardiyoloji Kliniği, bKadın Hastalıkları ve Doğum Kliniği, Anadolu Sağlık Merkezi, KOCAELİ Heart disease is among the most important causes of maternal death in pregnancy. Pregnant women with pre-existing heart disease should be managed in multidisciplinary teams of tertiary centres. The cardiac status of these patients must be optimized before delivery. Planned elective delivery is preferable, and vaginal delivery with careful anaesthesia is usually safe. In addition, incipient cardiovascular disease, including peripartum cardiomyopathy, myocardial infarction and aortic dissection, accounts for approximately one in six maternal deaths. A high index of suspicion, particularly in women with risk factors, early cardiology input, and invasive monitoring during labour and delivery are essential for their successful diagnosis and management. The serious consequences of poorly controlled heart disease in pregnancy could be minimized in medical centres capable of providing multidisciplinary support, and equipped with adequate systems for early detection and treatment. Keywords: Pregnancy, cardiovascular disease, laborTurkiye Klinikleri J Int Med Sci 2005, 1(50):64-68
