Management Of Pregnant Women With Cyanotic And Acyanotic Congenital Heart Disease

aKardiyoloji Bölümü, Dr. Siyami Ersek Göğüs Kalp ve Damar Cerrahisi Eğitim ve Araştırma Hastanesi, İSTANBUL Increasing numbers of patients with congenital heart disease are more frequently surviving into the childbearing ages as a result of advances in pediatric cardiology and cardiac surgery. Hormonally mediated increases in blood volume and heart rate, and decreases in systemic vascular resistance and arterial pressure result in a major increase in cardiac output during pregnancy. These hemodynamic changes may deteriorate the cardiac problems particularly in patients with cardiac shunts. Pregnant women with congenital heart disease are at risk for cardiovascular complications including heart failure, arrhythmia, stroke, neonatal complications, and in certain circumstances death. In most of the patients pregnancy is well tolerated. Some pregnancies may only be completed successfully with an acceptable risk following the correction of cardiac defect prior to conception. However in high-risk patients pregnancy should absolutely be discouraged.
