The Strategy Of Anticoagulation In Pregnant Patient With Prosthetic Heart Valve
Dr. Mehmet KANADAŞI,a Dr. Murat ÇAYLIa
aKardiyoloji AD, Çukurova Üniversitesi Tıp Fakültesi, ADANA Thromboembolic risk is high in pregnant patients with prosthetic heart valve. In these patients, hipercoagulable situation due to pregnancy physiology necessitates effective anticoagulation. Today, warfarin, unfractionated heparin (UFH) and low molecular weight heparin (LMWH) are used for anticoagulation. In addition to anticoagulation activity, the potential adverse effects of drug on patient, fetus or infant also should be bewared for selection of drug. Warfarin seems that the most effective therapy throughout pregnancy or during 2nd and 3th trimester for patients with high thromboembolic risk. Both UFH and LMWH which are recommended before 6th week for reducing embriopathy risk have high thrombosis risk than warfarin. Today, there is no consensus about LWMH dosing and assessing of LWMH effectiveness. Before selecting the anticoagulant drug, the patient risk must be determined. Then, the decision of proper anticoagulant strategy should be made after full discussion with the patient and her partners. Keywords: Pregnancy, prostethic heart valve, anticoagulationTurkiye Klinikleri J Int Med Sci 2005, 1(50):26-30
