Hypertension And Target Organ Damage

aKardiyoloji AD, İstanbul Üniversitesi Cerrahpaşa Tıp Fakültesi, İSTANBUL Hypertension is a major risk factor for cardiovascular disease, and also both systolic and diastolic blood pressures show a continious graded independent relationship with risk of stroke and coronary events. Primary objective of antihypertensive therapy is to reduce hypertension induced cardiovascular, serebral and renal mortality and morbidity. Recently published hypertension guidelines and expert reports leaded major changes related to the definition, risk stratification and treatment targets of hypertension. According to these reports although the decreasing the blood pressure to the optimal levels is still the primary objective antihypertensive therapy, only the level of blood pressure is not anymore accepted as a sole indicator of risk stratification and the introduction of antihypertensive therapy. Evaluation of cardiovascular risk factors and diabetes mellitus, target organ damage and hypertension related associated clinical conditions should also be the integral part of the risk stratification and the treatment. The most radical proposal is brought by the American Society of Hypertension (ASH) at the annual ASH meeting in 2005, regarding to the new definition of hypertension and risk stratification. This new proposal is primarily based on the view of hypertension as a complex cardiovascular disorder rather than as just blood pressure values. It characterizes the disease as a progressive cardiovascular syndrome with many causes that result in both functional and structural changes to the heart and vascular system.Keywords: Hypertension, target organ damage, vascular diseaseTurkiye Klinikleri J Int Med Sci 2005, 1(33):16-25
