Biventricular Pacemaker

aKardiyoloji AD, Kocaeli Üniversitesi Tıp Fakültesi, KOCAELİ Pacemaker treatment in heart failure which started as implantation of classical pacemakers in early 1990’s didn’t give favourable results. First left ventricular epicardial pacing was performed in 1995 during an open heart surgery. After its initial favourable hemodynamic results, clinical studies started in patients with dilated cardiomyopathy and intraventricular delay. Benefical clinical, echocardiographic and hemodynamic effects of biventricular pacing lead to a change in pacemakers guidelines and implantation of biventricular pacemakers has become a class IIa indication in patients with dilated cardiomyopathy and broad QRS complexes. Unfortunately, nearly 20% of the patients who received a biventricular pacemaker according to guidelines do not respond to this treatment. Recent studies revailed some predictive parameters fort he response to biventricular pacing.
Importance and application fields of biventricular pacing increases each day and important changes in classical pacemaker applications are expected.
