Unexplained Syncope

aKardiyoloji AD, Dokuz Eylül Üniversitesi Tıp Fakültesi, İZMİR Detailed medical history and physical examination findings particularly focused on structural heart disease provide the most valuable information for the evaluation of syncope. Togeteher with these findings, ECG and orthostatic blood pressure as well as carotid sinus massage (in the elderly patients) consist the "initial evaluation" of syncope. Following the initial evaluation data can be obtained for a certain diagnosis. If a suspected diagnosis exists, relevant diagnostic tests should be used. When the etiology of syncope is unexplained, probability of a heart disease (including rhythm and conduction abnormalities) which is the most important prognostic predictor should be initially examined. Long term ECG recording particularly with implantable loop recorders seems to be helpful and appropriate tool for this aim. Neurally mediated syncope is the responsable mechanism in the other major group of unexplained syncope, thus tilt table testing should be considered for the next step.Keywords: Unexplained syncope, diagnosisTurkiye Klinikleri J Int Med Sci 2005, 1(15):61-66
