Sudden Death And Risk Determination In Ventricular Arrhythmias

aKardiyoloji AD, Erciyes Üniversitesi Tıp Fakültesi, KAYSERİ Three million people die due to sudden cardiac death (SCD) per year. Thus to determination the people at high risk for SCD and to manage according to risk factors are main goals. Advanced age, mail gender, smoking, obesity, diabetes, hypertension, hyperlipidemia and emotional stress are well known risk factors. The other high risk indices are reduced ejection fraction of left ventricle, high classes of NYHA functional status and inducibility of ventricular arrhythmias at electrophysiological study (EPS). In comprehensive studies implantable cardiac defibrillators (ICD) were found to be effective that have left ejection fraction below 30% and have inducible ventricular arrhythmias at EPS. Coronary angiography must be performed at risk determination of patients with coronary artery disease. The risk determination of patients with reduced left ventricular ejection fraction and inducible ventricular arrhythmias may be reached with ECG based investigations that include premature ventricular beats and variability R-R intervals on Holter records, T wave alternans on various sites of heart measured with techniques which signal handling at level of microvolt and QT dispersion on different ECG derivations. Genetic and molecular investigations as well must be performed after echocardiographic, catheter and electrophysiological studies at patients without organic heart disease.
